ABSTRACT

Context: Private practice dentistry is predominant in Nigeria. Audits of fixed-prosthodontic practice in these settings are rare and will highlight capacity and guide training. This study aimed to assess the scope and quality of fixed-prosthodontic practice in such settings in Nigeria. Objectives: The study sought to assess: Participants' sociodemographic and their diagnostic and preoperative practices. The quality of impression making and operative technique, reviews and maintenance, temporization practices and communication with the laboratory.The range of treatments given and provision of advanced treatments. Materials and Methods: An adapted questionnaire was administered electronically on Nigerian dentists practicing fixed-prosthodontics in private practice settings. Retrieved data underwent descriptive statistics and associations were tested with the Fischer's Exact and Chi-Square tests using IBM SPSS Statistics version 21. Statistical significance was set at p≤0.05. Results: A total of 100 valid responses with a male:female ratio of 2.1:1 from 16 states wereretrieved. Fifty-nine participants had only first degrees and mean experience was 14.3±9.5 years. There were 27 adequate responses with regards to range of treatments offered. Sixty-three participants practiced direct temporization. Twenty-one and 14 participants regularly practiced implantology and CAD/CAM dentistry respectively. Direct temporization was significantly associated with increasing education (X2=6.03, p=0,05) and experience (X2=13.2, p=0.03). Conclusion: Only a few Nigerian dentists in private practice gave an adequate range of treatment. Most of them prefer direct temporization. Implantology and CAD/CAM dentistry practice are improving, but are still not very common.

ABSTRACT

@#This study is aimed to provide an insight into the types of treatment carried out by postgraduate students in the past 10 years. A retrospective study was carried out between November 2019 and February 2020 by deriving data from postgraduate students’ clinical logbooks of three different cohorts of Master of Clinical Dentistry (Restorative Dentistry in Conservative Dentistry) in 2010, 2013 and 2016. This retrospective data was systematically extracted from the records and tabulated categorically using an Excel spreadsheet (Microsoft Office Excel 2019). A descriptive analysis was carried out to further evaluate the results. Overall, the types of treatment carried out in the field of endodontics and fixed prosthodontics has changed in the past 10 years. An increasing trend is seen in the placement of all ceramic crowns and primary root canal treatment procedures, while a decreasing pattern is evident for secondary root canal procedures and placement of dental bridges.

ABSTRACT

OBJECTIVES@#To analyze the clinical performance of the intraoral digital impression (IDI) in the fixed prosthodontics.@*METHODS@#Databases of Medline (Ovid), EMBASE, Cochrane Central Register of Controlled Trials, and CNKI were searched for randomized controlled trial (RCT) on the use of IDI in fixed prosthodontics until May 2020. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. A Meta-analysis was conducted when available.@*RESULTS@#Eleven RCTs involving 618 patients were included in this study. A total of 2 and 3 studies had low and high risks of bias, respectively, and other included studies had a medium risk of bias. Results illustrated that the IDI group could shorten the impression-taken time [SMD=-5.63, 95%CI (-11.25, -0.01), @*CONCLUSIONS@#Evidence indicated a good clinical performance of IDI for fixed prosthodontics. Notably, high-quality studies are expected to further support the conclusion.

ABSTRACT

El objetivo de este estudio fue evaluar parámetros clínicos en Prótesis Fijas de Resina Compuesta Reforzada con Fibra (PFs-RCRF), en pacientes de la Clínica de Prostodoncia Fija de la Universidad Católica del Uruguay. Se realizó un estudio observacional, longitudinal, analítico. Se seleccionaron 25 pacientes entre 18 y 65 años con ausencia de una pieza posterior y con antagonistas naturales, restaurados o no. Las PFsRCRF fueron realizadas por docentes, y estudiantes de 5° año bajo supervisión docente. Un observador calibrado (Kappa 0.88) controló cada 6 meses durante 5 años y evaluó tomando como referencia criterios de Ryge G (1980) simplificados. Se consideró Alfa (A: sin alteración), Beta (B: alteraciones que no requieren tratamiento) y Charlie (Ch: necesidad de reciclado o recambio). Se controló Tinción Marginal (TM), Integridad Marginal (IM), Superficie (S) y Color (C). Se hicieron 10 controles en 20 PFs-RCRF y los resultados promedio de los retenedores fueron: TM: A: 47,6%, B: 48,3%, Ch: 4,1%; IM: A: 33,6%, B: 61,8%, Ch: 4,6%; S: A: 62,4%, B: 32,5%, Ch: 5,1% C: A: 73,7%, B: 23,6%, Ch: 2,7%. Los resultados promedio de los pónticos fueron: S: A: 59,7%, B: 34,9%, Ch: 5,4%; C: A: 90,1%, B: 6,9%, Ch: 3%. Se concluye que las PFs-RCRF han demostrado en 5 años, ser una alternativa funcional, estética, rápida, reciclable, de bajo costo y relativamente buena confiabilidad. Son fáciles de remover y reemplazar si fuera menester. Son importantes los controles periódicos para el éxito de dichas estructuras.


The aim of the study was to assess clinical parameters in Fixed Prosthesis of Fiber-Reinforced Composite (FPs-FRC) in patients of Fixed Prosthodontic Clinic of the Universidad Católica del Uruguay. An observational, longitudinal and analytic study was carried out. Twenty-five patients between 18 and 65 years old with a missing posterior with natural antagonists restored or not, were selected. The FPs-FRC were produced by teachers, and 5th year students – teaching supervision. One calibrated observer (Kappa 0,88) controlled every 6 months during 5 years and assessed taking as reference the simplified criteria of Ryge G (1980). The following was considered: Alpha (A: no alteration whatsoever), Bravo (B: minor alterations not requiring treatment) and Charlie (Ch: recycling or replacement needed). Marginal Staining (MS), Marginal Integrity (MI), Surface (S) and Color (C) were controlled. Ten controls were made in 20 FPs-FRC and average results of retainers were as follows: MS: 47,6%, B: 48,3%, Ch: 4,1%; MI: A: 33,6%, B: 61,8%, Ch: 4,6%; S: A: 62,4%, B: 32,5%, Ch: 5,1%; C: A: 73,7%, B: 23,6%, Ch: 2,7%. Average results of pontics were the following: S: A: 59,7%, B: 34,9%, Ch: 5,4%; C: A: 90,1%, B: 6,9%, Ch: 3%. It is concluded that, FPs-FRC proved to be in 5 years, functional, aesthetic, quick, recyclable, low-cost and with relatively good reliability. They are easy to remove and replace if necessary. Periodic inspections are important for the success of these structures

ABSTRACT

This clinical audit is aimed to provide an insight into the performance of dental technicians in renderingfixed prosthodontics services at Faculty of Dentistry, University of Malaya. A retrospective audit wascarried out between 1st of November 2014 and 31st January 2015 using data derived from records andmonthly returns of the technicians, which are kept at the ceramic laboratory. Retrospective data oncases of diagnostic wax-ups, full metal crowns, metal ceramic crowns, all ceramic crowns and bridgesthat were sent to ceramic laboratory for fabrication from 1st of September 2013 to 31st of August 2014was systematically extracted from the record and tabulated categorically in SPSS version 22.0. Theturnaround time in workings day for diagnostic wax-ups and the prostheses was calculated by deductingexit date from entry date. Subsequently, the turnaround time and the complexity of cases were categorizedaccordingly. The association of turnaround time and the complexity of the cases was analysed usingFisher Exact test with p value < 0.05. Within this time frame, a total of 102 cases of diagnostic waxups,36 cases of crown and 18 cases of bridges were fabricated. 57.8% of diagnostic wax-ups werecompleted within 3 days. 100% of 1 unit crown were completed within 7 days and 94.4% of bridges werecompleted within 14 days. There was a significant association of turnaround time and the complexity ofthe cases for diagnostic wax-ups and crowns with p value <0.05. The standard for turnaround time isbeing met by the ceramic laboratory at Faculty of Dentistry, University of Malaya. However, due to theexcessive workload, the overall output of all the measured procedures remains low.

Este estudo in vivo destinou-se a analisar as alterações histológicas que ocorrem na interface osso/implante durante o período crítico após a colocação em carga imediata. O modelo experimental foi desenvolvido na mandíbula de seis cães Beagle, tendo sido colocados dois implantes cilíndricos no terceiro e quarto quadrantes, respetivamente (n=24 implantes). Os implantes no quarto quadrante, em cada animal, foram reabilitados dentro do período de 24 horas, e o outro par, do terceiro quadrante, foi usado como controle, sem reabilitação. As eutanásias ocorreram aos nove, 17 e 31 dias após a colocação em carga, tendo sido preparadas amostras por técnicas não descalcificadas. A relação entre as medições do contato ósseo no terceiro e quarto quadrantes e as forças de mastigação foram analisadas estatisticamente no tempo de experimentação. O contato osso/implante foi ligeiramente maior, sem ser signifi cativo, nos implantes em carga imediata (52,13%), do que nos implantes dos quadrantes utilizados como controle (47,6%). O contato ósseo mostrou uma tendência para um valor menor no segundo grupo, aos 17 dias, e maior no primeiro grupo. Na comparação de contato osso/implante entre os quadrantes, apenas o terceiro grupo apresentou aumento do contato osso/implante no quarto quadrante. This in vivo study aimed to analyse the histological changes that occur in the bone-implant interface during the critical period following immediate loading. The experimental model was developed in the mandible of six Beagle dogs, having been placed two cylindrical implants in the 3rd and 4th quadrants, respectively (n=24 implants). The 4th quadrant implants, in each animal, were rehabilitated within a period of 24 hours and the other pair, the 3rd quadrant ones, was used as a control without prosthetic rehabilitation. Animals were sacrificed at 9th, 17th and 31st days after loading and samples prepared using non-decalcified techniques. The relationship between bone contact measurement at the 3rd and 4th quadrants, as well as masticatory forces, was statistically analysed in the time of the experiment. The bone-implant contact was slightly higher, without statistical signifi cance, on the immediate loaded implant group (52.13%) than in the quadrants used as controls (47.6%). The bone contact showed a trend towards lower values in the 2nd group (17 days), and higher in the 1st group. When comparing bone-implant contact between quadrants, only the 3rd group showed increased bone-implant contact at the 4th quadrant. Las expectativas de los pacientes en relación con el resultado estético de las prótesis fijas requiere una visión amplia del tratamiento del profesional. Esto incluye los aspectos dentales y de las estructuras que dan armonía a la sonrisa. Este trabajo tiene por objetivo mostrar a través de un caso clínico, el acondicionado mucoso mediante la técnica de escarificación para el restablecimiento satisfactorio de la armonía dento gingival, lo que permite la formación de las papilas interdentales y la conformación del arco cóncavo mucoso para la correcta instalación de los fantomas. La técnica se mostró con mucha previsibilidad cuando está indicada, planeada y ejecutada de la forma correcta, siendo una opción válida de tratamiento para el clínico obtener resultados estéticos satisfactorios


The patients' expectations related to the aesthetic outcome of dental prosthesis requires a broad view of the professional treatment. This includes all aspects of the teeth and the structures that create harmony in the smile. This paper aims to show through a case report utilizing the conditioning of the soft tissue (gingival scarification), a possible way to achieve a satisfactory restoration of the gingival tooth harmony, allowing the formation of the interdental papillae and the gingival concave arc shape for the proper installation of pontics. The technique is very predictable when directed, planned and executed correctly, being a valid treatment option for the clinician to obtain satisfactory aesthetic results

Introducción y Objetivo: El éxito se define comúnmente como Supervivencia los factores significativos que la influencian.estan relacionados con el paciente, con el implante o con la restauración. El propósito del este estudio fue evaluar la supervivencia de los implantes colocados en la clínica CES Sabaneta entre el 2000 y el2007. Materiales y Métodos: Se tomaron datos de las historias clínicas, se reclutaron el mayor número de pacientes y se evaluaron clínica y radiográficamente Resultados: Entre 2000 y el 2007 se colocaron en la institución 144 implantes en 81 pacientes el (63,9%) fueron mujeres tres casas comerciales principalmente, Nobel Biocare (38,9%), 3i (32,6%) y Lifecore (22,9%). Dado que hubo un alto número de pacientes que no pudieron ser contactados, se asumió el tiempo hasta la fecha de última revisión del implante, como tiempo de supervivencia del mismo y se estableció la diferencia en tiempo de uperviviencia de los implantes en estos pacientes con respecto a la supervivencia de los implantes en los pacientes contactados, no se encontraron diferencias estadísticamente significativas en el promedio del tiempo de supervivencia de los implantes. El promedio en el tiempo desde la colocación de los implantes hasta el análisis fue de 5 años aproximadamente, el éxito fue del 89,6%, es decir de los 144 implantes colocados se presentaron 15 fracasos. Conclusión: La supervivencia de los implantes colocados en la Clínica CES Sabaneta, está de acuerdo a los reportes en la literatura tanto de instituciones educativas como privadas.


Introduction and Objetive: The success of dental implants is commonly defined as survival and there are significant factors that influence it. Some of these are related to the patient, others with the implant and finally with the restoration. Materials and Methods: The purpose of this study was to evaluate the survival of implants placed in the CES University Dental Clinics between 2000 and 2007. Results: Data was collected from the medical records of as many patients possible and found that during this period 144 implants in 81 patients were placed. Most patients were female (63,9%), Implants placed corresponded to Nobel Biocare (38,9%), 3i (32,6%) and Lifecore (22,9%), and the remaining percentage from other commercial brands. Conclusions: Average time from implant placement until the analysis was approximately 5 years with a success rate of 89,6%, which means that out of 144 implants presented 15 were missing. It can be concluded that the survival of implants placed at CES University Dental Clinics coincides with reports in the literature as well as private educational institutions.

ABSTRACT

This study was to evaluate the clinical application of thinner ceramic veneer restoration. Tooth color was measured by eye and colorimeter after thinner ceramic veneer restoration. The results showed that there were many factors influencing the color of its clinical restoration. Hue of the ceramic veneer, ceramic materials and bond composites had greater influence than that of dentition. The brightness of ceramic materials and bond composites was closely correlated with the whole restoration brightness. A closer correlation was found with the brightness of bond composite. Thinner ceramic veneer restoration can achive satisfactory results.
